Role Overview

• Location: Albany, New York
• Full-time 5 day/week schedule 
• Post-Acute care inpatient setting
• On-call during the day Monday through Friday

What You'll Do

• Deliver comprehensive primary care
• Manage acute and chronic conditions
• Develop personalized care plans
• Integrate and work closely with the IDT
• Partner and work collaboratively with (Login for Information) physician

Who We Are Looking For

• Experienced clinicians
• FNP, CNP, AGNP, APRN, or PA
• Active New York NP/PA license
• Active DEA 

Compensation
Compensation is fee for service (FFS). Expected compensation is estimated salary range of $160,984 to $191,181 annually with no cap on productivity income potential with full benefits.
