- Key Responsibilities Serve as Medical Director of Record for Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs across two long-term care facilities in South Carolina. Review, sign, and maintain medical orders for chronic-care patients enrolled in RPM/CCM. Collaborate remotely with facility nursing staff and the TRC care coordination team to ensure compliance with CMS and facility policies. - Qualifications Active South Carolina medical license (MD or DO) Background in Family Medicine, Internal Medicine, or Geriatrics preferred Board Certification preferred but not required Experience with chronic-care coordination, telemedicine, or long-term care a plus Strong communication skills and comfort working with remote teams

Description:
Total Remote Care, LLC is seeking a South Carolina–licensed physician (MD/DO) to serve as Medical Director solely for the purposes of Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) oversight. This is an administrative position with no direct patient care or call responsibilities. The physician will review and sign work orders for enrolled chronic-care patients, maintain clinical oversight of remote monitoring protocols, and collaborate with facility nursing staff and remote care teams to ensure compliance with CMS and facility policies.

Skills/Requirements:

Active South Carolina medical license (MD or DO)
