--Practice Setting: Office with rotating call obligations with the hospital
--Dates of Coverage: Start by 11/14
--Locums not Responsible for Supervising Other Health Care Providers
--Nursery Level: III
--EMR: Epic
--Estimated Credentialing Timeframe: 2.5-3 months
--Schedule: Monday-Friday (8:30-4:30); Call Ratio 1:3
--Average Patients Per Day: 10-15 depending on visit type
--Three Physicians in practice; Nursery Level 3; Onsite lab; Onsite ultrasound

Requirements:
--Board Certified/Truly Board Eligible
--Fellowship training in Maternal Fetal Medicine
--Active New York State License
--BLS/ACLS
--CDS Certification and DEA Registration
