Lead Nephrology Specialist
(Login for Information) is seeking a dedicated and experienced Lead Nephrology Specialist to join our partner, a dynamic medical center in Newark, NJ. This is an exciting opportunity to contribute to an innovative value-based care model, providing exceptional kidney care through a hybrid approach of in-home visits and telehealth.
Job Description:
As a Lead Nephrology Specialist, you will play a pivotal role in delivering patient-centered care to individuals with chronic kidney disease. Your responsibilities will include:
- Providing comprehensive nephrology care in both in-home and telehealth settings.
- Managing and coordinating patient care across various settings, including inpatient, outpatient, and dialysis.
- Collaborating with interdisciplinary healthcare teams to ensure optimal patient outcomes.
- Utilizing value-based care models and quality improvement methodologies to enhance patient care.
- Participating in population health management initiatives.
- Utilizing strong clinical skills to diagnose and treat patients with diverse kidney diseases.
- Promoting cooperative decision-making and effectively communicating with patients and their families.
Requirements:
- Medical degree from an accredited medical school.
- Board certification in Nephrology.
- Current state medical license and DEA registration without restrictions.
- Strong clinical skills and experience in caring for patients with kidney diseases across a variety of settings (inpatient, outpatient, dialysis, etc.).
- Commitment to providing patient-centered care and promoting cooperative decision-making.
- Excellent communication and interpersonal skills, with the ability to effectively collaborate with patients, families, and healthcare teams.
- Experience with value-based care models, quality improvement methodologies, and population health management initiatives preferred.
