Job Description

Seeking a Nurse Practitioner to provide effective and equitable value-based primary care to Medicare patients in Fayetteville, North Carolina. Details: Full-time, Mon - Fri, 8am-5pm 13-16 patients per day Geriatric population Provision of exceptional primary care. Conduct office visits for routine and acute issues. Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation. Care coordination with other providers, specialists, testing facilities, and agencies. Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years) Assisting the care team with phone triage and outreach. Conducting home visits as needed Qualifications: Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate required National certification as a Family, Adult or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC Experience in primary care: Internal medicine, geriatrics, or family medicine preferred Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).
