Job Title: Nurse Practitioner-Primary Care
Location: Youngstown OH
Immediate opening for an experienced Nurse Practitioner working in Primary care in Youngstown.
Full time position - normal M-F schedule but there may be very occasional consultative calls on the weekends, but expect that to be light to non-existent.
Competitive salary and 10% bonus, 2K allowance for continuing ed and recertification
What you will be doing:
You will see patients with Serious Mental Illness being responsible for supporting, caring for and documenting their medical needs. Performing a whole-person clinical assessment to identify and remediate care gaps. On an ongoing basis, you will support care coordination among the individual’s medical and behavioral health providers, making referrals as appropriate, as well as provide clinical support to Guides (peers) and Community Resources Guides (social workers).
Requirements for this opportunity: 3+ years clinical care as a Nurse Practitioner, seeking candidates with solid Primary Care exp.; Working with and managing underserved populations with challenges such as behavioral health and/or SUD conditions; Developing and delivering educational programs to nurses and affiliated medical staff; Care management and coordination; Current Ohio license
