Details/Requirements:
--Must be Board Certified or Board Eligible
--Fellowship: preferred
--Licensed in AZ or IMLC
--ACLS, BLS, PALS certified
--Temporary privileges available
--EMR System: MEDHOST, HMS, EDIS
--Bed In Department: 8
--Estimated Credentialing Timeframe (Days): 30
--Seeking to credential locum OB/GYN providers for future openings. High risk/Trauma patients will be transferred to a Trauma hospital  
--Call Type: Beeper
--Phone Consults: 1-2
--Call Response Time Required: 30 minutes
--Average number of deliveries per year: 390

Required Procedures:
--Low risk pregnancy
--High risk pregnancy
--FHR interpretation – NST (Non-Stress Test)
--FHR interpretation – CST (Contraction Stress Test)
--FHR interpretation – FSE (Fetal Scalp Electrode)
--OB ultrasounds – performance
--OB ultrasounds – interpretation
--Level I/screening (viability) ultrasound
--Level II/targeted (anatomic) ultrasound
--Amniocentesis
--Routine delivery
--Vaginal delivery
--Low forceps delivery
--Decision-making for cesarean section
--Complicated delivery
--Cesarean section
--Breech delivery
--Multiple births
--Abruptions
--VBACs (Vaginal Birth After Cesarean)
--Repair of 3rd & 4th degree lacerations
--Office/minor gynecology – IUD, cystoscopy, colposcopy, cryosurgery, LEEP, etc.
--Surgical gynecology
--Open surgical gynecology
--Laparoscopic diagnostic/minor procedures – tubal ligation, LOA's, chromotubation
--Operative/major gynecology (includes diagnostic) – ectopic pregnancy, ovary removal, cystectomy, extensive LOA's
--Laser certification
