Coverage Details
-- September 15
-- September 23
-- October 2, 8, 10, 14, 20, 21, 24
-- November 3, 4, 5
-- December 29 – December 31, 7 AM – 7 AM
-- Locums will NOT be responsible for supervising other providers

Shift Description
-- 24-Hour On Call
-- Pager Call
-- Very low volume with rare Call Back

Requirements
-- Board Certified
-- Active Oregon license
-- DEA and CSR
-- BLS
-- Local candidates – highly preferred
-- Must submit proof of COVID vaccine in order to work at this facility
