What You’ll Do
- Provide primary and continuous medical care to PACE participants
- Collaborate closely with Nurse Practitioners and the interdisciplinary team
- Coordinate care with specialists, hospitals, and skilled nursing facilities
What We’re Looking For
- MD or DO; Board Certified in Internal Medicine or Family Medicine
- Active Missouri Physician License, DEA, and CDS
- Experience caring for an elderly population
- Passion for team-based, value-driven care
