Job Description

ESSENTIAL DUTIES AND RESPONSIBILITIES: 

  • Provides services to persons with severe and persistent mental illness who qualify for services under the Intensive Community Outreach and Recovery Team (I-CORT) program and serves as a member of the I-CORT multidisciplinary team.
  • Provides services as specified on the weekly schedule. Team members have the ability to provide a variety of functions and must demonstrate a willingness to be flexible in their duties.
  • Provides support to individuals, both on a planned and as-needed basis, to help them accomplish their personal goals, to cope with the stressors of day-to-day living, and to reach goals as identified on the ISP and Recovery Support Plan.
  • Provides nursing assessments in compliance with DMH standards and within the scope of Mississippi Nursing Practice Acts and Rules and Regulations.
  • Evaluates client’s response to medications i.e. effectiveness and/or side effects and documents appropriately.
  • Administers medication injections as prescribed by physician/PMHNP and follows all other orders.
  • Provides general health care monitoring, supportive counseling, recovery/resiliency support, and symptom management.
  • Provides services in the individual’s natural/community setting 75%-85% of the time.
  • Participates in daily organizational meetings to review service contacts within the past 24 hours and action plan for the current day.
  • Participates in agency MAP teams and staffings when indicated.
  • Maintains client confidentiality at all times.
  • Documents appropriate activities in case note format that justify the need for treatment.
  • Maintains case record documentation that is adequate, appropriate, complete, and timely.
  • Completes and submits service logs per agency policy.
  • Develops the Recovery Support Plan with input from the multi-disciplinary team. Assists in development of the Individual Service Plan and Crisis Support Plan.
  • Responds to clients needs in an appropriate and timely manner.
  • Demonstrates appropriate interactions with coworkers that promote a pleasant work environment.
  • Completes additional job duties assigned by supervisor.
  • Participates in interagency collaboration for the provision of client services when assigned.
  • Maintains licensure and seeks professional clinical training opportunities.

EDUCATION AND QUALIFICATION REQUIREMENTS:

Required

  • Completion of an accredited nursing program
  • Licensed to practice as a Registered Nurse in the state of Mississippi.
  • Satisfactory completion of CPR prior to reporting to the work site.
  • Satisfactory completion of CPI within 30 days of employment.
  • Valid driver’s license and auto liability insurance.
  • Ability to transport clients in private vehicle when necessary.

Preferred

  • Previous experience in a mental health setting strongly preferred.
  • Previous experience working with an SMI population in a treatment setting strongly preferred.
