Hospital is seeking a Family Practice Physician that specializes in Sports Medicine.

The following is a list of the compensation and benefits to be included in an employment contract.

Â?    Term    Twenty-Four (24) Months w/ Annual Renewals
Â?    Base Compensation    $212,000 Base Salary
Â?    Productivity Package    WRVUÂ?s > 3,968 Produced in the Family Practice Office
Paid at $30.00
Â?    Health Insurance    Covered by the Practice for Participation in H.S.A. Plan
Â?    Disability Insurance    $6,000 per Year for Documented Expenses
Â?    Dues/Membership/CME Fees    $5,000 per year for Documented Expenses
Â?    Malpractice Liability    Covered for Active Physicians; Tail Prorated Based on     Length of  Service
Â?    Personal Leave    4 weeks (20 days) Paid Time off for Sick/Vacation Time
Â?    CME Leave    2 weeks (10 days) Paid Time Off for Educational Purposes
Â?    Pension Plan    401(k) With a 2.5% Match capped at IRS Limit
Â?    Work Schedule Expectation    40% Student/Athlete Health; 60% Family Practice
