Your residency established the standard of care.  This fellowship trains the surgeons who will redefine it. Endoscopic spine surgery is the fastest-growing frontier in operative spine care, yet high-volume, hands-on endoscopic training remains incredibly rare in traditional academic settings.

At Excel Neurosurgery & Spine, we are bridging that gap.

We are currently seeking one exceptional, board-eligible Orthopedic or Neurological Surgeon for a rigorous, 12-month Endoscopic Spine Fellowship. Based entirely in our state-of-the-art Ambulatory Surgery Center (ASC), this program is designed to transform competent graduates into elite, independent endoscopic specialists.

Through intensive dry-labs, cadaver training, and high-volume clinical cases, you will master the transforaminal and interlaminar approaches with both uniportal and biportal techniques.  Progress from targeted lumbar discectomies/decompressions to cervical/ thoracic, and ultimately, endoscopic fusions (Endo-TLIF).
  • The Business of Endoscopy: Mastering the scope is only half the battle; getting paid for it is the other. Unlike traditional academic fellowships, we teach you the exact Revenue Cycle Management (RCM) strategies required to run a private practice. You will learn how to navigate unlisted CPT codes, win peer-to-peer reviews, and negotiate commercial ASC carve-outs.
  • The Outpatient Edge: Learn how to safely and efficiently transition complex, multi-level spinal pathology out of the hospital and into the ASC using awake and enhanced recovery (ERAS) protocols.

Program Details & Requirements

  • Duration: 12 Months (August 1, 2026 – July 31, 2027)
  • Location: Honolulu, HI
  • Eligibility: Must have successfully completed an ACGME-accredited residency in Orthopedic Surgery or Neurological Surgery and be eligible for a Hawaii medical license.
  • Compensation: Competitive PGY-level salary, comprehensive malpractice coverage, and full benefits.
