Urgent Care Provider (3 FTE in FTW & 2 FTE in Dallas)

We are looking for an Urgent Care provider (3 FTE in FTW & 2 FTE in Dallas) for service.

- Start date: 12/01/2016

- End date: Ongoing

- Location: Dallas/Ft. Worth

- Type of Clinic (MSG, SSG, Solo, CH): SSG

- Schedule: M-F: 8am-10pm, Sat: 8am-8pm, Sun: 8am-5pm.

- Patient Ages: 50% Adult, 30% Geriatric, 20% Pediatric

- IP/OP: Outpatient

- On call: No

- Support Staff: Yes

- Charting/Dictation: EMR

- BC / BE Requirement: Board Certified or Board Eligible

- Specialty: Family Medicine

- DEA / CSR Requirements: Yes

- Certifications: BLS

- Position: 15183

- Current State Medical License
